I really don't know how it is decided who goes on the spine. Actually it is interesting that no homunculus besides Bradley has appeared on the spine. And then when you think about it, Bradley actually plays a dual role since he is also the Fuhrer (of course in the volume that he is on the spine is when he was revealed to be Wrath).
I think there is going to be about 20 volumes, every character won't even have a chance to appear on the spine at this point even ones that we feel are deserving.
I think that the spines are reserved for the most important characters. Edward, Alphonse, Roy, and Scar are all givens, for example, as is Winry, Izumi, and Ling. That said, I'd interpret Bradley's appearance on the spine as a signal that he is one of the central antagonists, who will be focused on the most, and so far I've got to say, personal fandom and biases aside, that Bradley is the most well developed of all the antagonists, in terms of depth, personality, and general characterization.
That said, I think we can expect that all major players will eventually gain their own spines, although perhaps not in the order we expected. Still, I suspect Envy and Father will each get one, maybe Greed and Kimbley.
